Clinical Information

This assay is a screen for recent oral DOAC (Xa inhibitor) medication use, prior to starting 
therapeutic heparin. It is not a DOAC (Xa inhibitor) drug level. Direct oral Anti Xa inhibitors 
include Apixaban (Eliquis) and Rivaroxaban (Xaralto). DOAC screen testing should be negative, 
<0.10. If positive, ≥ 0.10, provider and or pharmacy should consider switching to UFH anti-IIa for 
monitoring heparin therapy for 72 to 96 hours depending on renal clearance.

Ordering Instructions

The DOAC Screen test should be ordered prior to initiating heparin therapy. Utilize if the presence of Direct oral Anti Xa inhibitors are suspected. Direct oral Anti Xa inhibitors include Apixaban (Eliquis) and Rivaroxaban (Xaralto).

Specimen Collection: Blood

 

Container(s): Blue top (3.2% Na. Citrate)

Preferred Volume to Collect: 2.7 mL (Full 2.7 mL blue tube)

Minimum Volume to Collect: 1.8 mL (Full 1.8 mL blue tube)

Neonate Volume to Collect: See minimum volume

Processing Instructions (Laboratory, Outpatient or Off-site collection)

  • Processed Specimen: Double Spun Platelet Poor Plasma 
  • Centrifuge/Spin: Yes
  • Aliquot: Yes
  • Processing Instructions:
    • Detailed instructions (link): Preparing Platelet Poor Plasma
    • Separate plasma into 2 - 3 aliquots, minimum of 0.5 mL each.
    • Freeze aliquots in plastic transport tubes immediately.
  • Transport Temperature:
    • Ambient (up to 1 hour in whole blood state)
    • Frozen (plasma only >1 hours)

Specimen Stability

  • Ambient whole blood state unspun: 1 hour
  • Ambient spun: 4 hours
  • Frozen:  Aliquot and freeze double spun platelet poor plasma within 1 hour of collection
  • Frozen at -70°C: 2 months
  • Frozen at -20°C: 2 weeks

Test Frequency

Test run Daily.  TAT 4 hours
