Clinical Information

Corewell Health West uses the reverse algorithm for syphilis testing. 

Syphilis Total Antibody Screen (LAB1230501) is the recommended order for syphilis testing. Specimens that test positive for syphilis total antibody will automatically reflex to RPR. 

If the Syphilis Total Antibody and RPR results are discordant (i.e. Syphilis Total Antibody=reactive and RPR=nonreactive), the specimen is sent to MDHHS for TP-PA testing (Treponema pallidum particle agglutination) to adjudicate the discordant results.

Exception: GEO location orders RPR Only.

Preferred Volume to Collect: 5.0 mL

Minimum Volume to Collect: 2.0 mL

Neonate Volume to Collect: 1.0 mL

Collection Instructions:

  • Specimen should be processed within 2 hours.
  • Invert tubes 8-10 times and allow to clot 30 minutes before centrifuging.
  • Avoid hemolysis.
  • Refrigerate after centrifuging.

Methodology

RPR  is a non-treponemal agglutination test for the serologic detection of Syphilis. Carbon-particle cardiolipin antigen detects "reagin", a substance present in serum from syphilitic patients, and occasionally in persons with other acute or chronic conditions.
