Clinical Information

Cord blood samples are tested to determine the ABO/Rh type of the infant and to detect the presence of ABO, Rh, or other antibodies potentially involved in hemolytic disease of the newborn (HDN).

 

Cord blood specimens may be contaminated with Wharton's jelly and other cord contaminants. The red cells from cord specimens must be washed thoroughly before they are tested. Newborns do not have their own ABO antibodies, therefore ABO reverse type is not performed.

 

Providers will order a Cord Blood Workup on the infant in the hospital information system, including the entry of the mother’s MRN as part of the order which can be helpful to determine if HDN testing should be performed. It is also used to compare identifying information on the specimen label.

Specimen Collection: Cord Blood

Preferred Volume to Collect: 3.0 mL

Minimum Volume to Collect: 1.0 mL

Neonate Volume to Collect: 1.0 mL

Collection Instructions:

  • Blood is collected from the infant’s umbilical cord at the time of delivery and is placed in a labeled screw top specimen vial or EDTA.
  • Gently invert EDTA tube(s) 8 - 10 times after collection.
  • LABEL
    • Label must have the infant's name, MRN, collection date and the user ID's of the person labeling the specimen and the witness.
    • Specimens for Blood Bank must have 2 signatures on the specimen written in ink that is resistant to smearing
    • A "CORD BLOOD" label may also be placed on the specimen
  • Non-EDTA cord blood specimens are considered clotted specimens.
  • If cord blood sample volume is insufficient for testing, mislabeled or no cord blood sample is available, request a Newborn Blood Bank Workup order and a heel stick specimen collected.
